Key Policy Benefits of FREE COVID-19 Insurance for Amazon Sellers

Note:
- There is a 15-day waiting period (from the date of issuance) before the coverage starts. Acko will not be liable to make any payment under the applicable benefits if the Coronavirus Disease (COVID-2019) is found positive prior to the coverage start date.
- The coverage start date will be 15 days after the date of issuance of cover (since there is a 15-day waiting period).
Eligibility Criteria
Only sellers who fulfill the following eligibility criteria can avail this offer of Free COVID-19 Insurance Policy by Amazon:
- Sellers who have had at least 1 active listing on Amazon.in anytime between 1st Jan 2019 and 26th May 2020. (irrespective of when it was created)
- Sellers between the age of 18-60 years.
- Sellers who have submitted valid personal information on the opt-in form.

Once you fill the form, Amazon will contact you within 48 Hrs to update you about the status of your application and the next steps.
If you are found to be eligible, the copy of the cover under the policy will be issued to you within 72 hours of the close of the enrollment window.

Steps To Apply For FREE COVID-19 Insurance
- Fill out your personal details on in application form. (Read Complete T&Cs before you apply)
- Email the following documents to in-sellerinsurance@amazon.com:
- Scanned copy of your GST certificate/ Business PAN card
- Scanned copy of identity proof for the policy holder (Voter ID/ PAN card/ Aadhar card/ Passport/ Driving license)
- Amazon will reach out to you with the status of your application and the next steps within 48 hours after you fill the form.
NOTE:
1. The coverage will be provided to the individuals of Age between 18- 60 Years.
2. The claim will be payable only if your test for Coronavirus Disease (COVID-2019) is found positive for the first time, after 15 days from the coverage start date.
3. Only one person per seller account can opt for this free insurance cover.
What is Excluded from the Policy
Amazon Website lists the following exclusions. You may not be able to claim the benefits in case of the following conditions.
- Acko shall not be liable to make any payment under the applicable Benefits, if the Coronavirus Disease (COVID-2019) is found positive prior to the coverage start date.
- Claim will not be payable, if the testing is done in a centre other than ICMR Authorized test Centre in India: https://icmr.nic.in/sites/default/files/upload_documents/Testing_sites_for_COVID19.pdf
- Treatment taken for any illness, sickness or disease other than the Novel Coronavirus (nCov) (COVID-19)
- Acko shall not liable to pay claim if insured person doesn’t follow the international travel advisory related to Coronavirus (COVID-19) issued by the Indian Government (MoHFW).
- Any pre-existing condition is not covered whether declared or undeclared.
- Any Symptomatic treatment in absence of Coronavirus Disease (COVID-19) is not covered.
- Treatment taken outside India is not covered.
Claim Process
You need to file a claim with Acko General Insurance Limited though Family Health Plan Insurance TPA Limited (FHPL).
Below are the details to claim the benefits of FREE COVID-19 Insurance Policy in case the policy holder is tested positive for Coronavirus.
You can file both cashless as well as reimbursement claims.
Process to File Cashless Claim
Following are the steps to file a cashless claim that you can opt for at any of our 5000+ Network Hospitals:
Step 1: Share your UHID no, inform help desk/insurance helpdesk at the hospital that your claim will be handled by FHPL (TPA) and Acko General Insurance Limited Gen insurance is your insurer.
Step 2: Fill and sign the form provided by the hospital desk and submit it at the helpdesk.
Step 3: Once you provide all the details on the form hospital desk will send the cashless request to FHPL for further processing.
Step 4: FHPL will either approve the cashless request, or may raise additional information or may reject the case.
Step 5: All the updates on the cashless facility will be shared with you on SMS and email.
Step 6: If your cashless facility is denied, you can still lodge the claim for reimbursement.
The list of network hospitals keeps on updating frequently in view of which we request you to kindly visit the TPA website www.fhpl.net and choose hospital networks and select the insurer as Acko General Insurance and check the updated list of Network hospitals.
How to File Reimbursement Claims
You can initiate a claim through FHPL at acko.crm@fhpl.net with the following details in the subject line of the email- Amazon Seller: (NAME); Contact No and the scanned copies of the original documents.
Process for scanning the documents:
- You will need to scan the following original documents (refer to the list of mandatory documents below).
- Before scanning, please write with a pen “FOR ACKO INSURANCE” on the ORIGINAL documents and then scan the same.

- Claim form duly filled and signed by the insured
- Original Discharge summary
- Original Death Summary (in case of death)
- Original hospital bill with detailed break-up of charges applied by hospital
- Original payment receipts with receipt numbers & stamp/ seal of the provider
- Original Pharmacy/ medicine receipts with receipt numbers & stamp / seal of the provider
- Original Invoice/Stickers/barcode in case of implants
- Laboratory and test reports
- First consultation paper from doctor stating the origin duration and progress of illness
- FIR/ MLC certificate (Accident claims)
- Medical prescription/ Prescription
- Cancelled cheque of the proposer with Name of the client/ Bank Name / IFSC code /MICR code and account number or First page of passbook with Name of the client/ Bank Name/IFSC code/ MICR code and account number
- Aadhaar card, or any other government photo ID and PAN Card.
- Other documents as may be required by Acko General Insurance to determine the admissibility of claim.
- Certificate from the treating doctor stating the circumstances due to which domiciliary treatment was administered (for domiciliary hospitalization claims only)

Yes. In case of a positive COVID-19 diagnosis, the insured person will be eligible for claim. However, only COVID-19 related hospitalization expenses up to INR 50,000 is covered under the policy. Any pre-existing condition is not covered whether declared or undeclared and any symptomatic treatment in the absence of COVID-19 is not covered.
No, you cannot make modifications to your cover under the policy once you have submitted the form. However, if you need to update details such as your address, you can reach out to Acko General Insurance at 1800-266-2256 for assistance.
